I hereby undertake that neither I nor any other person in my house/workplace (family members/guard/maid/servant/staff) has been deducted as Corona positive in last 15 days and I'm not suffering from Fever, cold, cough & throat pain at the time of entering the salon. I have authorised Salon to sanitize me with 70% Alcohol based Sanitizer at the time of entry in the salon. During my visit the salon has followed social distancing and carried out sanitization process.
